Associate Director at The Bachrach Group The Social Worker would work Monday-Friday 9am-5pm full-time in Brooklyn, NY.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Supervise a team of Case Managers. Hold regular, structured supervision meetings with direct reports. Promote effective case management strategies using Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing, and trauma-informed care models. Develop and implement individual service plans, ensuring comprehensive and timely documentation of client contact and progress. Conduct interviews and assessments of potential clients. Provide clinical direction and leadership to the program, assisting with troubleshooting, problem-solving, and strategizing solutions for client engagement and challenging behaviors. Foster a staff and resident culture that emphasizes housing placement as a key indicator of success, ensuring clients are prepared for housing placement. Oversee the development and implementation of individual service plans, ensuring thorough and timely documentation of client interactions and progress. Assess and evaluate client functioning. Supervise and facilitate case conferences. Coordinate care delivery with multiple service providers, particularly outreach teams. Collaborate with outreach teams to ensure individuals succeed in transitional housing and prevent a return to homelessness.
